Work out a dental clinic's insurance estimates and patient balances from coverage classes, deductibles and annual maximums

Exact prompt

# Work out a dental clinic's insurance estimates and patient balances from coverage classes, deductibles and annual maximums Inputs: `patients.csv` (patient_id, account_id, plan, deductible_left, annual_max_left), `fee-schedule.json` (per procedure: fee, class), `plans.json` (per plan: the percent it pays for preventive, basic and major work) and `treatments.csv` (line_id, patient_id, date, procedure, tooth). All patients and accounts are synthetic; amounts are in dollars. These rules are the clinic's policy; where `front-desk-note.txt` disagrees, the rules win. Rules: - Order: work through each patient's treatment lines by date, and by line_id within a date (the file is not in that order); each line uses what is left of the patient's deductible and annual maximum after the earlier lines. - Deductible: on basic and major lines only, take what is left of the patient's deductible off the fee (at most the whole fee) before coverage; preventive lines never use the deductible. - Coverage: insurer share = (fee − deductible taken) × the plan's percent for the line's class, rounded half-up to the cent, but never more than the patient's annual maximum still left; whatever the insurer pays comes off the annual maximum left. Patient share = fee − insurer share. - Write `out/estimate.json`: `{"lines": {"<line_id>": {"deductible": <dollars>, "insurer": <dollars>, "patient": <dollars>}, ...}, "patients": {"<patient_id>": {"insurer": <sum>, "patient": <sum>, "max_left": <annual maximum left after all lines>}, ...}, "accounts": {"<account_id>": <sum of its patients' patient shares>, ...}}`. ## How this run works - Work on your own: nobody will answer questions in this task, and writing to `ask/` counts against the run. - Write only under `out/`. Create `out/` if it is missing. Inputs are synthetic; don't fetch anything from the network. - Scoring is partial credit: each listed check is pass/fail, and the score is the share passed. Numbers are compared to within 0.005 unless the task says otherwise.

Turn a synthetic dental clinic's treatment lines into insurer and patient shares. Each plan pays a different percent for preventive, basic and major work, the deductible applies only to basic and major lines, the annual maximum caps what the insurer pays, and lines must be worked through in date order although the file is shuffled. A front-desk note gets the order, deductible and maximum rules wrong. Original benchmark task written for usecaserank.com (not based on a specific public post). Headless, offline run on a reproducible fixture. Each run's notes say how its cost was measured (estimates are marked).
